- Original International Certificate of Vaccination for Yellow Fever
- Copy of airline tickets or travel itinerary
- TVS Request Form completed
business visa
- Valid Passport (Must have at least 6 months validity remaining)
- 2 Application forms – completed and signed
- 2 Passport-type photographs
- Original International Certificate of Vaccination for Yellow Fever
- Copy of round airline tickets or travel itinerary
- A business letters of responsibility is required, to be typewritten on your company letterhead and addressed to:
- “Embassy of Ghana, Visa Section, Houston, TX.”
- TVS Request Form completed
