- Valid Passport (Must be valid 6 months beyond intended stay)
- 1 Application form – completed and signed
- 1 Passport-type photograph
- Copy of round trip airline tickets or itinerary
- Letter of Invitation from company in Cambodia on company letterhead
- A business letter of responsibility is required, to be typewritten on your company letterhead and addressed to:
- “Embassy of Cambodia, Visa Section, Washington DC.”
- TVS Request Form completed
