Tourist Visa

  • Valid Passport (Must be valid for 6 months beyond intended stay)
  • 1 Application form – completed and signed
  • 1 Passport-type photographs
  • Copy of travel itinerary
  • Copy of recent bank statement or evidence of adequate finance
  • Hotel confirmation or letter of invitation
  • TVS Request Form  completed

 

business visa

  • Valid Passport (Must be valid for 6 months beyond intended stay)
  • 1 Application form – completed and signed
  • 1 Passport-type photograph
  • Copy of travel itinerary
  • A business letter of responsibility is required, to be typewritten on your company letterhead and addressed to:
    • “Embassy of Bolivia, Visa Section, Washington DC.”
  • TVS Request Form completed
